How to Fill Out an Oregon Practitioner Credentialing Application

This blog post provides a step-by-step guide on how to fill out an Oregon practitioner credentialing application. The information in this post is based on the video "How to Fill Out an Oregon Practitioner Credentialing Application" by Jennifer Blevin Smith, MD, JD, MBA.

Steps to Follow

  1. Read the instructions carefully. Before you begin filling out the application, it is important to read the instructions carefully. Make sure you understand all of the requirements and deadlines. It is also a good idea to keep an unsigned and undated copy of the application on file for future reference.
  2. Gather the required documents. The required documents typically include your state professional license, DEA certificate, CV, and malpractice insurance certificate. Be sure to gather all of the required documents before you start filling out the application.
  3. Fill out the application completely and accurately. Be sure to fill out all of the sections of the application completely and accurately. Be sure to initial and date each page.
  4. Pay attention to detail. Do not skip any sections, even if they do not apply to you. If a section does not apply, mark it "N/A".
  5. Attach additional sheets if necessary. If there is any information that you cannot fit on the application form, you can attach additional sheets of paper. Use a separate sheet of paper to explain any incomplete programs or other relevant information.

Additional Tips

  • Use a personal email address for the application. This is how you will receive important updates and notifications about your application status.
  • Keep an updated CV on hand. This will make it easier to fill out the application, as you will not have to spend time gathering all of your information.

Conclusion

Filling out a practitioner credentialing application can seem like a daunting task, but it does not have to be. By following the steps outlined in this blog post, you can ensure that your application is complete and accurate.

PART 2:

This blog post is about how to fill out a credentialing application for the state of Oregon, but the information in this application is going to be the same information collected no matter which state you're going to be submitting a credentialing application to.

Here are the steps on how to fill out a credentialing application:

  • Fill out everything from the first page and the first sections to section 13.
  • Section 14 is about health care licensures, registrations, and certificates. You will need to fill out your state's license number, what type of licensure it is, and the expiration dates. If you are a prescriber and you have a DEA number, you will need to include that as well.
  • Section 15 is about your individual NPI number, your Medicare number, and your state Medicaid number.
  • Section 16 is for other state health care licenses, registrations, or certificates.
  • Section 17 is about hospital and other healthcare facility affiliations. You will need to list all of the hospitals and other healthcare facilities where you have or have held clinical privileges and/or staff membership.
  • Section 18 is for professional practice and work history. You will need to list all of your previous employers, as well as your current employer. You will also need to include your professional liability carrier for each of these positions.
  • Section 19 is for peer references. You will need to list at least three people who can attest to your clinical skills and competence.
  • Section 20 is for CMEs. You will need to list all of the CMEs that you have completed in the last two years.
  • Section 21 is for professional liability share insurance. You will need to list your current professional liability insurance carrier, as well as any previous carriers from the last five years.
  • Section 22 is for attestation questions. You will need to answer all of the attestation questions yes or no.
  • Section 23 is for the authorization and release of information form. You will need to fill this out if you are working with an IPA or another organization to help you with your credentialing.
  • Attachment A is for professional liability claims or lawsuits. You will need to list any past or current professional liability claims or lawsuits that have been filed against you.

Once you have completed all of these sections, you will need to go back through the application and make sure that everything is filled out correctly. You will also need to compare the application to your CV to make sure that everything matches. Once you are satisfied with the application, you can save it and submit it.

Here are some tips for filling out a credentialing application:

  • Collect all of the information you need before you start filling out the application.
  • Go through the application first and write out anything that you still need outstanding from the practitioner.
  • Fill out the application uninterrupted so that it can be thoroughly and efficiently filled out.
