Speech Therapy Billing Company
Speech Therapy
billing and RCM services
Speech therapy spending does not stand alone. Medicare adds it to the same patient’s physical therapy charges under one shared cap, and crossing it changes what every claim needs. Our speech therapy billing services track that number correctly, for every patient, every time.

The code you bill constantly is the one about to change the most.
Six places speech therapy revenue goes missing
No plan exists for the proposed code change
A practice that waits until a new code structure takes effect to understand it will spend its first claims of that period correcting avoidable denials instead of collecting revenue.
The combined threshold was tracked as speech-only
Billing as though speech therapy spending exists in isolation from a patient’s physical therapy charges produces a number that does not match what Medicare is actually tracking.
AAC device services were billed out of sequence
The ongoing programming and therapy code depends on the evaluation code being on file first. Billed without it, the claim fails regardless of the work performed.
A swallowing study claim was missing its radiology half
A videofluoroscopic study requires documentation from both the SLP and the imaging side, and a claim missing either one does not stand on its own.
Multiple evaluation codes were billed without separate justification
Billing more than one of the distinct evaluation codes the same day without documentation supporting each as its own service invites a denial.
The GN modifier was missing
Without it, a claim does not identify itself as a speech-language pathology service under a plan of care, and processing does not proceed correctly.

What actually moved for this specialty this year
CMS formally proposed deleting the most-used treatment code in the specialty
In July 2026, CMS proposed deleting CPT 92507, the generic individual treatment code speech-language pathologists have relied on for decades, to be replaced with ten diagnosis-specific codes effective in 2027. This is a proposal, not yet a final rule.
The combined therapy threshold rose
For 2026, the KX modifier threshold for combined physical therapy and speech-language pathology spending rose to $2,480, continuing to accrue as a single shared number rather than being tracked separately by discipline.
New remote therapeutic monitoring codes became available for SLPs
Codes 98984, 98985, and 98979 were introduced, billable with the GN modifier, supporting monitoring of a patient’s communication or swallowing program outside of a direct treatment session.
Telehealth authority for core SLP services was extended
Coverage for telehealth delivery of core codes, including 92507, 92508, 92521 through 92524, and 92526, was extended through December 31, 2027.
Someone has to track a proposal before it becomes a rule
A code deletion proposed in the middle of one year and taking effect the next is exactly the kind of change that catches an unprepared practice during its first weeks of a new billing cycle.
In-house vs. outsourced
What one biller cannot track through a code transition like this
| The question | One in-house biller | TruCare |
|---|---|---|
| Combined PT/SLP threshold | Tracked as speech-only | Combined correctly, the way Medicare calculates it |
| AAC billing sequence | Sometimes billed out of order | Evaluation and therapy sequenced correctly |
| Swallowing study coordination | Claims stall between departments | Radiology and SLP documentation kept connected |
| Proposed code changes | Learned about from denials | Tracked and prepared for ahead of time |
| Multiple evaluation codes | Billed without separate justification | Documented individually, every time |
| GN modifier | Sometimes missing | Applied on every applicable claim |
| Cost shape | Salary, benefits, training, software | A percentage of collections, so it scales with volume |
| Staff absence | Billing slows across the practice | Coverage is handled without disruption |
| Regulatory change | Discovered through a denial | Tracked and built into process ahead of time |
| Visibility | A report assembled on request | Live, by service type and by threshold status |

Speech therapy billing saver guide
A practice collecting $710,000 saves up to $55,381 a year by outsourcing to TruCare
| In-house billing | With TruCare | Annual saving | |
|---|---|---|---|
| Dedicated billing staff | $73,060 | $0 | $73,060 |
| Billing software and clearinghouse | Varies | Included | — |
| Training, turnover, and coverage | Varies | Included | — |
| TruCare fee (from 2.49% of collections) | — | $17,679 | — |
| Total annual cost | $73,060 | $17,679 | $55,381 |
Example: a practice collecting $710,000 a year with one full-time biller, at TruCare’s starting rate of 2.49%. Biller cost uses the BLS May 2025 median wage for medical records specialists ($51,140), with benefits at 30% of total compensation. Software and training costs vary by practice and are not counted. We do not replace your front desk, we take the claims work off it. Larger practices are quoted on volume rather than at the starting rate, so a bigger practice should not scale this example up.

Talk to a speech therapy billing expertWhat a practice gains once it is ready for the change before it arrives
“We found out about the proposal to delete our most common code from a billing newsletter, almost by accident. It made us realize we had no actual plan for what happens when the code we use on nearly every claim simply stops existing. Having someone already tracking that before it becomes official changed how we think about the next year entirely.”
